A sliding scale is a flexible fee system. The fee that the client pays can “slide” up or down based on the client’s ability to pay. A sliding scale usually takes into consideration the client’s household income. All prices are subject to change without prior notice due to currency fluctuation, and/or unforeseen economic circumstances.

Psychiatrist Services
When a client requests or needs forms (disability forms, work forms, workmans comp) filled out by you or the office there is a fee for that. That fee is $15.00 dollars and must be paid prior to the forms being completed and sent.
